Specialist Community Public Health Nurse – School Nursing Team – Grade 5

As specialist practitioners, the School Nurse functions as both health promoters and health educators. The School Nurse works in partnership with many agencies, professionals and families to promote and protect the bio-psycho-social health and wellbeing of children and young people, in the developing years. Working to safeguard the needs of children and young people in Jersey, providing a fully confidential health service including: advice, guidance and support with physical and mental health; healthy lifestyle choices; childhood continence and toilet training; behaviour management and modification; and adolescent and sexual health; as well as providing specific support for families with complex needs within the school setting. The post holder will deliver and develop this specialist service for school age children and young people providing advice and intervention within a multi-disciplinary framework. This will involve developing and maintaining a high clinical profile and presence and acting as role model for other nursing staff.

hours: Full Time - 37.5 pw

salary: Grade 5 - ranging from £61,529 -£65,805 per annum

Role details

The successful applicant for this School Nurse role will be someone who is enthusiastic about collaborative working and is committed to be self-reflective. You will be a highly experienced practitioner within the multi-disciplinary team and will contribute to the development of a clinical service through involvement in standard setting and the development and dissemination of expertise within  clinical and education settings or at home.

Key Accountabilities:

Professional Responsibilities

This role come under the management of the School Nurse Team Lead and the Operational Lead for Child and Family Service FNHC.

The post holder will work as part of the School Nurse Team, delivering the universal, targeted, or specialist service to all school age children and young people, from the point of school entry until 19 years Promote the holistic health of the school aged population, thereby enabling them to achieve their full potential.  You will encourage children and young people to think about their health and become responsible for their own wellbeing as they progress through childhood and adolescence.

The role is varied and includes:

•   Keeping children and young people safe from harm and protecting them from abuse, in accordance with Jersey Safeguarding Partnership Board Child Protection Procedures and Family Nursing & Home Care policies

•   Offering health advice and universal health surveillance, incorporating early intervention and support to children, young people and their families. The School Nurse Team works in partnership with allied health professionals;  colleagues in education, social care other community services; CYPES, HCS, other statutory and voluntary sectors, children, young people and families

•   Reviewing the health status of children and young people and facilitating health care plans as required

•   Promoting early intervention and help to support children, young people and families to reach their full potential

 The post holder will promote a culture of evidence-based practice by contribution to developing and maintaining protocols, documentation, systems, standards, policies and guidelines in line with the Clinical Governance Framework and adhere to the standards set out by Jersey Care Commission.

You will demonstrate enhanced nursing practice and will act as a role model in terms of own care delivery by acquiring knowledge and skills to support a client based service supported by the multi-disciplinary team.  The post holder will support individual clients/carers through all aspects of treatment programmes, providing a ready means of access and advice.

The post holder will make a significant contribution to a specialty service through developing effective clinical leadership skills and will keep fully up to date in developments within own professional discipline and will share clinical knowledge/experience in order to enhance nursing care.  You will maintain a professional portfolio which demonstrates the continuing acquisition of skills, knowledge, attitudes, understanding and achievement.

The post holder will demonstrate a thorough knowledge of the guidelines of the NMC and Code of Conduct and Scope of Professional Practice and apply these to maintain trust and credibility with clients, carers and colleagues.

You will facilitate effective communication and liaison between hospital and community, with the use of agreed and existing appropriate systems, guidelines and protocols and pathways. Where necessary you will work to develop appropriate systems, guidelines, protocols and pathways.

You will work in partnership with Education and Health & Social Services to meet the needs of children, young people and families to enable access to The Right Help at The right time.

You will triage cases of children who have attended the Emergency Department (ED), are inpatients in Maternity, Special Care Baby Unit, both on and off island and are on the paediatric ward and with consent, which is obtained by HCS for all attendees, share proportionate information with FNHC Child and Family Services in a timely fashion, depending on the assessed level of urgency to meet care planning needs and readiness for discharge; attend Health Safeguarding Meeting, collate information for the meeting, liaise with GP’s, and the multi-agency team for clients and share outcomes from the meetings with relevant practitioners from HCS and FNHC, too increase communication and enable front line Child and Family practitioners to make their autonomous practice decisions.

You will access FNHC electronic records, analysing risk from assessment of records where appropriate and accurately record following recommended record keeping guidance using templates which enable data analysis; access hospital based electronic and if appropriate, paper records with inter-organisational access agreements to enable understanding and case management for liaison and follow FNHC safeguarding Adults and Children Policy and SPB Guidance on Making Safeguarding Personal.

The post holder will make a significant contribution to safeguarding through developing effective clinical leadership skills and Safeguarding Restorative Supervision (SRS).

Communication and Liaison Responsibilities

Establish and maintain effective liaison pathways with other agencies involved with the care of children e.g. Social Workers, GP’s, Schools and Nurseries, Police, Child & Adolescents Mental Health Services (CAMHS), Mental Health Services and Sexual Health Services.

Triage risk following guidance for children following the Was Not Brought Policy (HCS 2021).

MASH: MASH cover will be provided for FNHC in line with Commissioning agreement.

Children Looked After

The post holder will be responsible for completing health assessments and reviews for Children Looked After and will plan and deliver interventions as necessary.

Safeguarding

The post holder will take responsibility for the safeguarding and welfare of all children, adults and/or families within their care and conduct and must be familiar with the signs and symptoms of abuse and know what to do if any such concerns are raised or are identified.  He/she shall be aware of and comply with Family Nursing & Home Care safeguarding policies and procedures.

Identify children and young people in need of support and protection, acting in compliance with the Jersey Safeguarding Partnership Board Standards and Procedures https://safeguarding.je/

•   Refer to Child and Family Hub where safeguarding concerns are identified

•   Engage with Safeguarding, Child in Need and Early Help

•   Represent Community Specialist Child and Family Services FNHC in attending safeguarding planning meetings i.e. ICPC’s, RCPC’s, core group meetings and strategy meetings.

•   Liaise with specialist services

•   Working in collaboration with professionals to safeguard children to reduce risks
